About the Role
Join our IC Geriatrics team as an Occupational Therapist and help older people maintain independence, build resilience and achieve positive health outcomes within their community. You will deliver comprehensive assessments, coordinate care and support hospital avoidance through restorative, person centred interventions. Occasional weekend work is required.
• Assess needs and deliver tailored occupational therapy care
• Support hospital avoidance and safe supported discharge
• Coordinate care across community and aged care settings
• Collaborate with multidisciplinary and external care teams
• Build restorative and resilience focused care plans
• Support older people with complex health needs
About You
You are a compassionate and capable Occupational Therapist who delivers person centred care and works confidently within complex community and rehabilitation settings.
• AHPRA registration as an Occupational Therapist
• Deliver evidence-based care for complex client needs
• Communicate well with clients, carers and teams
• Manage priorities and work with professional autonomy
• Support quality improvement and multidisciplinary care
