Occupational Therapist – NDIS & Community Rehabilitation

The Opportunity

If you are an Occupational Therapist looking for flexible, well-supported community work where your clinical input makes a visible difference, this is an opportunity to join Vitalis Health on a casual basis.

You will deliver occupational therapy visits in the name of Vitalis Health to clients in their homes and communities. Work will be allocated where it makes practical sense, taking into account where you live, your availability, your clinical experience and the locations of our clients.

Your caseload may include NDIS participants, older people, clients with complex care needs and individuals recovering from illness, injury, surgery or hospitalisation. The focus is on practical, person-centred occupational therapy that improves safety, independence and participation in everyday life through functional assessment, rehabilitation, home modifications and assistive technology solutions.

Vitalis Health delivers specialised clinical and care services in the home. Our nurse-led, multidisciplinary model brings together nursing, physiotherapy, occupational therapy and community care to support people to recover, remain independent and live well at home.

As our allied health capability grows, we are building a trusted network of clinicians who value autonomy, responsive service and coordinated care. You will work independently in the community while remaining connected to experienced clinical and operational teams.

Your new role

As a Casual Occupational Therapist, you will assess clients in their own environment and develop practical, goal-oriented strategies aligned with their needs, routines and priorities. Depending on your experience and interests, the role may include:

• Functional, cognitive and occupational therapy assessments in clients’ homes.
• Activities of daily living (ADLs), functional capacity, home safety and independence support.
• Rehabilitation in the home following illness, injury, surgery or hospitalisation.
• Equipment prescription, assistive technology assessments and home modification recommendations.
• NDIS assessments, interventions, reports and collaboration with support coordinators.
• Falls prevention, environmental assessment and strategies to support safe ageing in place.
• Education and practical strategies for clients, families and carers.
• Coordination with nurses, physiotherapists, doctors and other health professionals.
• Accurate and timely clinical documentation, reports and progress notes.

What We Are Looking For

• A recognised qualification in Occupational Therapy and current AHPRA registration.
• Experience in community care, NDIS, rehabilitation, aged care, disability or complex care.
• Strong clinical reasoning and confidence working autonomously in clients’ homes.
• Clear communication, reliable documentation and a genuinely person-centred approach.
• A current Australian driver licence, access to a suitable vehicle and capacity to travel locally for client visits.
• Ability to meet relevant pre-employment and client-service compliance requirements, including NDIS Worker Screening and other checks where applicable.
